Topic: What does it mean to take responsibility for a mistake?

Consider one of the following mistakes, consider one of the entities responsible, and what it would mean to "take responsibility".

A security breach, such as Sony's or Anthem's.
A factory robot that crushes someone's arm.
A medical diagnostic program that does not diagnose a rare but curable condition in time to save the person's life.

You may refer to any ethical theories or anything else you read in the textbook.
